Three consecutive sentences of 29 CFR 1926.1205 carry three different subjects, and the third is not a person. Before entry is authorized, "each entry employer must document the completion of measures required by § 1926.1204(c) by preparing an entry permit."1 Before entry begins, "the entry supervisor identified on the permit must sign the entry permit to authorize entry."1 The third sentence begins with the document. The completed permit "must be made available at the time of entry to all authorized entrants or their authorized representatives, by posting it at the entry portal or by any other equally effective means."1 It closes by saying why: "so that the entrants can confirm that pre-entry preparations have been completed."1

Sixteen items, and what the rule accepts in place of a name

The permit "must identify" sixteen things, running from paragraph (a) through paragraph (p) of 29 CFR 1926.1206.2 Among them is "Each person, by name, currently serving as an attendant."2 Among them also is "The individual, by name, currently serving as entry supervisor, and the signature or initials of each entry supervisor who authorizes entry."2 The authorized entrants are handled differently. They may be identified "by name or by such other means (for example, through the use of rosters or tracking systems)."2 Any substitute has to let the attendant determine "quickly and accurately, for the duration of the permit, which authorized entrants are inside the permit space."2

A stated duration and a stated retention period

"The duration of the permit may not exceed the time required to complete the assigned task or job identified on the permit."1 The entry supervisor "must terminate entry and take the following action when any of the following apply," and three circumstances follow.1 One is completion of the entry operations, and the action is cancellation. In the other two a condition arises that the permit does not allow, and the action is cancellation in one and suspension or cancellation with a full reassessment in the other.1

Afterward, "the entry employer must retain each canceled entry permit for at least 1 year to facilitate the review of the permit-required confined space program."1 The written program carries a requirement of its own. It "must be made available prior to and during entry operations for inspection by employees and their authorized representatives."3

A plan written for named work at a named site

One sentence of 29 CFR 1926.502(k) sets two conditions on the fall protection plan option. The first is the kind of work: leading edge work, precast concrete erection work, or residential construction work.4 The second is that those employees "can demonstrate that it is infeasible or it creates a greater hazard to use conventional fall protection equipment."4 Where both hold, the plan "shall be prepared by a qualified person and developed specifically for the site" where that work is performed.4 The sentence ends "and the plan must be maintained up to date."4

The lead standard requires a document to be revised on a schedule. "Prior to commencement of the job each employer shall establish and implement a written compliance program to achieve compliance with paragraph (c) of this section," and the plan runs to nine lettered items.5 "Written programs must be revised and updated at least annually to reflect the current status of the program."5 The first of the nine describes each activity in which lead is emitted.5

Two training records that look alike

An employer verifies compliance with the fall protection training requirements "by preparing a written certification record."6 That record "shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer."6 Where the training was conducted by another employer, or completed before the section took effect, the record instead indicates the date the employer determined it was adequate.6

Confined space training records also carry three items. They "must contain each employee's name, the name of the trainers, and the dates of training."7 Both records name the person trained and the dates. Where one asks for the signature of the trainer or the employer, the other asks for the name of the trainers, and a single form cannot collect a signature and a name in one field.

What the citation record shows, and what it does not

OSHA publishes the standards cited most often by industry. For establishments classified in Construction of Buildings, federal jurisdiction, October 2024 through September 2025, the agency recorded 2,215 citations across 990 inspections.8 Section 1926.503, "Training requirements," drew 92 citations across 92 inspections.8 The table counts sections rather than paragraphs, so it does not show how many concerned the certification record.

The limits of an editorial reading

A safety consultancy decides whether a control is adequate. We are not one, and we do not tell a client whether a document satisfies a standard. Hazard, feasibility, and adequacy are settled before the copy reaches us.

An editorial reading asks narrower questions. Does the word attendant carry one meaning in the program, on the permit, and in the training material? Does a plan written for one site name that site? Does a form leave room for each item the program under it lists? Work sent to us is kept in confidence.

Paragraph 1926.1205(f) has the entry employer retain each canceled entry permit "for at least 1 year to facilitate the review of the permit-required confined space program."1 One reader is at the portal, confirming pre-entry preparations. Another opens the same page a year later. The same page has to answer both, and only the first reader was ever at the space.

A worked example: Safety Training Record

Training certification record, construction site safety program

Two training records on a construction site look almost the same and are not. Under 29 CFR 1926.503(b)(1) a fall protection certification record carries the name or other identity of the employee trained and the date or dates of the training. It also carries the signature of the person who conducted the training, or the signature of the employer. Under 29 CFR 1926.1207(d) a permit-required confined space training record carries each employee's name, the name of the trainers, and the dates of training. Both records name the person trained and the dates of the training. Where one asks for the signature of the person who conducted the training, or of the employer, the other asks for the name of the trainers. A single form headed Training Record cannot ask for a signature and a name in the same field. The extract below is reproduced unchanged in both panels, and every field name and retention period in the revision comes from it. Whether a given session was adequate training is determined by the employer, and that determination is not made or reviewed here.
