A failure mode analysis is meant to find the problems before customers do. Done properly it is one of the most valuable exercises in manufacturing. Done as a compliance exercise the week before a customer audit, it becomes a spreadsheet of two hundred rows in which every risk priority number lands just below the threshold that would require action — which is not a coincidence, and every quality engineer in the industry knows it.

The process FMEA is where manufacturing quality is either designed in or documented away, and the writing failure sits in three fields. Occurrence is rated from what the team hopes rather than what the data shows; detection is credited to a control that would not actually detect that failure mode; and severity, which is the one rating that cannot be improved by tinkering with the process, is quietly softened until the arithmetic behaves. We work through these so each failure mode is stated as what the process does wrong rather than as a defect name, since "porosity" is a symptom and "insufficient degassing time before pour" is a mode with a cause attached; so the effect is written from the point of view of the person who suffers it, at the end customer and at the next operation, because those two are often rated very differently and only one is usually recorded; so every detection control names what it detects and when — a first-off inspection does not detect a failure that starts in hour three; so occurrence ratings are tied to evidence, whether internal reject data, warranty history or a comparable process, rather than to a shared feeling; and so any action carries an owner, a date and the rating it is expected to change. Analyses written this way find things, which is the entire purpose.
